What this code covers

Source · Editorial summary grounded in 3 cited references ↓

72125 is non-contrast CT of the cervical spine. It is the workhorse study for neck trauma and the default when a surgeon needs to see bone: fracture lines, facet anatomy, ossified ligament, fusion mass, or hardware position.

In an ortho or spine practice, it shows up after a fall or motor vehicle accident, before posterior cervical fixation to plan screw trajectories, and months after an ACDF to judge whether the fusion took. Sagittal and coronal reformats are part of the study and are not billed separately.

Two things cause denials. First, commercial plans often require prior authorization through a radiology benefit manager for outpatient CT, and the authorized code has to match what was performed. If the order said with contrast and the tech ran it without, the claim fails. Second, diagnosis coding has to support CT over plain films. A neck pain code alone rarely holds up. Fracture, radiculopathy with failed conservative care, myelopathy, or postoperative assessment codes do.

Work RVU vs. total RVU

The work RVU (0.98) is the surgeon's own effort — the figure physician pay and productivity targets are built on. The total RVU (3.91) adds practice overhead and malpractice, and is what drives the Medicare payment below.

Payment by site of service

Medicare pays different rates by setting. HOPD typically pays substantially more than ASC for the same procedure.

Common denial reasons

The recurring reasons claims for CPT 72125 get rejected.

Source · Editorial brief grounded in CMS NCCI edits, AAOS coding appeals, and cited references ↓

  • No prior authorization or authorization issued for a different code
  • Diagnosis does not justify CT (nonspecific neck pain)
  • Contrast status on the claim does not match the report
  • Duplicate billing with a 3D rendering code without separate workstation documentation
  • Missing interpretation for the professional claim

Frequently asked questions

Source · Generated from the editorial pipeline, verified against 3 cited references ↓

01Is 72125 with or without contrast?
Without contrast. 72126 is with contrast and 72127 is without contrast followed by contrast.
02Can I bill reformats separately with 72125?
No. Standard multiplanar reformats are included. 3D rendering codes are separate only when the 3D work meets the code requirements and is documented.
03Does CT cervical spine need prior authorization?
Most commercial plans require it for outpatient CT, usually through a radiology benefit manager. Traditional Medicare does not use prior authorization for this study.
04What diagnosis supports CT of the cervical spine after fusion?
Codes for the fusion status or a complication such as suspected pseudarthrosis or hardware problem, along with the presenting symptom, make the medical necessity clear.
