What this code covers
Source · Editorial summary grounded in 3 cited references ↓
Three or four studies total. A tight single-limb entrapment exam can fit here, for example median motor, median sensory, and an ulnar sensory comparison for a straightforward carpal tunnel question.
Study counting decides the code. Count motor and sensory studies of the same nerve separately, and count an H-reflex as its own study. An F-wave rides with its motor study and does not add one. Do not count repeat stimulation sites on the same nerve as extra studies. A median motor study recorded from the wrist and elbow is still one study.
The code is billed once per date of service. If needle EMG was done, add 95885 or 95886 per limb. Contractors that follow a maximum study table expect certain counts by diagnosis, and a carpal tunnel case often supports more than four, so make sure the code reflects what was actually done.
RVU & reimbursement
Component RVUs and Medicare national rate. Actual payment varies by GPCI locality.

Work RVU vs. total RVU
The work RVU (1.22) is the surgeon's own effort — the figure physician pay and productivity targets are built on. The total RVU (3.54) adds practice overhead and malpractice, and is what drives the Medicare payment below.
| Work RVU | 1.22 |
| Practice expense RVU | 2.26 |
| Malpractice RVU | 0.06 |
| Total RVU | 3.54 |
| Medicare national rate | $118.24 |

Payment by site of service
Medicare pays different rates by setting. HOPD typically pays substantially more than ASC for the same procedure.

Common denial reasons
The recurring reasons claims for CPT 95908 get rejected.

- Repeat stimulation sites counted as separate studies
- Billed with multiple units
- Paired with limb needle EMG codes instead of add-ons
- Study count mismatch on audit
